 A maintenance technician arrived at a pump station to replace a defective valve. The Job Safety Analysis (JSA) had been completed, the Permit to Work (PTW) was approved, and the toolbox talk had already taken place, so everything appeared safe from a documentation perspective. However, conditions changed before the task began. A light rain made the metal grating slippery, tools left near the work area created a tripping hazard, and a nearby line remained live. None of these hazards were identified in the JSA or highlighted in the PTW.
